Physician's Permission Form: This form must be completed by your physician, and e-mailed, or presented in person at the time of your appointment if you have/had major skin, bone, blood, nerve, or organ conditions, surgeries or other major health conditions, or complications. Those with major health conditions may not receive services without this form.
